Your local authority will assess the level of support you need and this may be one to one support or shared support. You may require support with personal care and to do the activities you like.
The care and support services are provided either by Oakfield Care & Support, or by other providers of your choice.
If you choose Oakfield Care & Support Service
We will provide the support you are assessed as needing when you need it which will be provided by our highly trained group of staff. You will have a named Key Worker, who will work with you to develop your skills, wishes, ambitions and social interaction. Information will be circulated to the support team so that everyone is aware of your choices, decisions and wants, as well as what you do not like or would prefer not to take part in. You will be supported by a team of staff, who will work with you and support you within the home and out in the community.
All staff go through a rigorous recruitment and inductions process, completing mandatory training during their 3-month probationary period and regular on-going refresher training. In addition, all staff will be trained for any particular need that is identified, for example epilepsy and diabetes. Our staff receive regular supervision and assessments of their work to ensure they are competent to provide your support.
You will have a detailed support plan. It is important to us that you and your family are fully involved in developing your support plan, as it belongs to you. We will also consider any risks and will do all we can to reduce these, so you can live as full a life as you choose. Where there are any decisions to restrict your freedom or choices, we will ensure we do this correctly and in line with the requirements of the Deprivation of Liberty Safeguards under the Mental Capacity Act 2005.
To ensure the support plan remains relevant and up to date, we will hold a circle of support meetings every 3 months. You can invite who you wish to attend the meeting, but it is important to us that we get all the feedback we need to provide you with the best support.
As Oakfield is a community, we hold monthly “Being Heard” meetings. This is where you have another opportunity, along with other people we support, to raise any issues or suggestions you may have. You can tell us what you are happy with and what needs to change.
We are registered with the Care Quality Commission (CQC) to provide personal care, and accommodation with personal care at both of our locations. Please use the below links to view our latest reports:
